As a result of changes under the Paperwork Burden Reduction Act, CCMCF will no longer be automatically providing copies of Form 1095-C to employees.

Instead, all current CCMCF employees, and former employees, must request a copy of Form 1095-C by mail, phone, or email by contacting Payroll below. When you issue your request, please also advise your consent if you wish to have your form delivered electronically.
