At Calhoun County Medical Care Facility (CCMCF), we invite prospective patients and their families to consult with our Admissions Coordinator or one of our social workers to discuss purposeful living solutions provided to persons who need assistance. Whether the need is long-term medical care, short-term rehabilitation, or adult day care services, we'll discuss the specific care requirements and arrange a tour of our facility. This is a great opportunity to understand how we can meet your needs and ensure your comfort and well-being.
We provide 24-hour professional skilled nursing services to our residents. Our RN and LPN Charge Nurses, Clinical Nurse Managers, Certified Nursing Assistants, and on-site Nurse Practitioner provide outstanding medical care to our residents, in conjunction with our Medical Director.

What Do I Need For Admissions?
Our knowledgeable Admissions Staff is here to assist you with the application process and ensure a smooth transition into our facility. They are well-versed in Medicare and Medicaid guidelines and maintain close collaborations with hospitals throughout West Michigan. To help facilitate the admissions process, we will need:
From Your Physician
- History and physical (including height and weight within the last 60 days)
- List of current medication (with dosages)
- Mental Health Screening Form
- Any recent labs
From Your Family
- Photo ID, social security card, and insurance cards
- DPOA, Patient Advocate, or Guardianship paperwork
- Two main contacts (including name and phone)
- Vaccination and booster card/paperwork
