ADMISSION

Admission to our nursing home community will be without regard to race, religion, national origin, color, age, sex, handicap, sexual preference, marital status, and with particular but not exclusive concern for those county residents with limited financial resources. (According to state statute, however, a person must be at least fifteen years of age to reside in the facility.)

Information Required for Admission

Admission to the community begins with the placing of a potential resident on our waiting list. The following articles are required for admission:

• History and Physical, including height and weight

• List of current medications, including dosage

• Chest x-ray within the last 90 days

• Copies of recent laboratory analyses

• Mental Health Level I Screen (DSS 3877 & 3878)

• Copies of insurance cards

• Copies of Power of Attorney or Guardianship papers

• Copy of Social Security Card

• Physical/Occupational Therapy notes if available

Your physician should be able to provide these materials for you. They may be mailed or faxed to the community. After the above requirements are met, each prospective resident is evaluated by our Interdisciplinary-team for appropriate medical placement. Admission is always dependant on bed availability.
