ADMISSION
The Center provides quality Adult Day Care for persons who are socially
isolated, depressed, physically frail and developmentally disabled. We specialize in care
for adults who have Alzheimer's disease, Parkinson's disease, head injuries, postoperative
brain tumors, or are recovering from a stroke.
Information Required for Admission
Prospective clients are encouraged to call to arrange a visit
to the Center prior to admission for a tour
and the completion of the required paperwork. If it is not possible, the
Director will
make a home visit.
The following items are required for admission:
- History and Physical, including height and weight
- List of current medications
- Chest x-ray within the last 90 days
- Copies of insurance cards
- Copies of Power of Attorney or Guardianship papers
After the above requirements are met, the client may expect to be enrolled in the
program within approximately two weeks.
